Former Green Beret John Rambo is serving time in a federal prison. When the US military hears of American soldiers missing in action from the Vietnam war possibly still being alive and held captive, Colonel Trautman is authorized to send Rambo in to rescue them. For Rambo, this is what he would do best. If only the pencil-pushers would stop getting in his way.

While in jail, a deputy takes delight in abusing him. Rambo escapes showing his old Vietnam fighting skills and takes to the woods as the sheriff and deputies try and find him in his element. Things get out of hand as Colonel Trautman Rambo's old commander appears to shed light on the situation.

John Rambo's former Vietnam superior, Colonel Samuel Trautman, has been assigned to lead a mission to help the Mujahedeen rebels who are fighting the Soviet invasion of Afghanistan, but the Buddhist Rambo turns down Trautman's request that Rambo help out. When the mission goes belly up and Trautman is kidnapped and tortured by Russian Colonel Zaysen, Rambo launches a rescue effort and allies himself with the Mujahedeen rebels and gets their help in trying to rescue Trautman from Zaysen.
